A few notes about all orders:

• Deadline dates will be on the calendar as well as in the newsletters and on the white board in the studio.
• Orders may not go in immediately.  We need to collect a few orders to keep prices where they are unless specified.​
• Forms can be printed, filled in and brought in to the studio with payment.
• Orders will not be submitted without payment.
• All forms and designs have been created by and are property of Sara Kate's Dance Studio and cannot be copied or used for any other purpose without our permission.

Dance Shoes Order Form:

• Ballet shoes will be ordered in pink
• All other shoes will be ordered in tan
• Team members need to order specific shoes (all on the right side of the flyer)
• All other students may order any shoes (even those marked team) except lyrical. half sole.
• These shoes need to be ordered in time to be used for competitions and recital.
• Order dates will be announced on the news page.

Studio Gear Order Form:

• Several different types of bags to be personalized with your first or last name.
• Click on the studio gear link for pictures and the order form link to print out the form.
• All order forms and payments need to be in by 8:00 on the specified dates to be included.

Costume Deposit Forms

• Deposits will be due in November.  The date will be on the calendar and on the white board in the studio.
• A $35 deposit must be made for each costume that is ordered.

• All shirts come in black and are in unisex sizes.
• Available in T-shirts, Hoodies & Full Zip Hoodies
• Baseball T's have specific colors.  See order form.
